Job Description
Minimum Qualifications:
Bachelor’s degree from an accredited university or college in an approved human services field such as social work, elementary education, special education, nursing, psychology, counseling, sociology, speech and language pathology or equivalent approved field of study or a Bachelor’s degree in a non-related field with at least nine (9) college hours of related course work in the fields listed above. Be able to complete the required WV Birth to Three training hours to be credentialed as an Interim Service Coordinator. Valid driver’s license & dependable transportation with minimum automobile liability insurance as established by the Board of Directors and be willing to travel extensively throughout the region. The individual selected for the position must also be willing to work non-traditional hours with a flexible schedule. The successful candidate must be self disciplined, highly motivated, submit to a background check and drug test and be bondable along with being proficient with all Microsoft Office Based Programs and also Computer capable.
Job Synopsis:
Follow up on all child referrals. Conduct family interviews, gather child and family information, obtain relevant authorizations and releases. Aid the family with any initial linkages and referrals they may need. Assist with eligibility determination, facilitate and participate in a multi-disciplinary team and help with the development of the initial Individualized Family Service Plan. Schedule IFSP team meetings, work with families throughout referral, Eligibility and IFSP development and provide and maintain accurate child/family documentation and IFSP changes. The ISC will also assist with minor problem/complaint resolution, participate in staff development plan and on-going training including training on children with disabilities and perform any and all duties incidental to the position. The successful candidate must also be credentialed each year through the WV Birth to Three CFO, COVANSYS and submit to an annual Criminal Background Check.
This position will be located in RAU 6, the successful candidate will complete home visits in Braxton, Greenbrier, Monroe, Nicholas, Pocahontas, Summers and Webster counties and housed out of either Lewisburg or Beckley.
