Contractors Approval Process & Requirements


MountainHeart Community Services, Inc. maintains an "open entrance/open exit" system to assure that contractors who meet the minimum requirements  will be approved as sub-contractors to complete residential and commercial building trades construction projects on behalf of MountainHeart.  All approved contractors are encouraged to perform work for other companies and members of the public.  In no instance will an approved contractor be considered as an employee for any purpose.

MountainHeart publishes in the Mullens Advocate and Independent Herald newspapers a classified advertisement solicity contractors one (1) day per month.  The advertisement will direct all initial inquires to the website,  The CONTRACTORS APPROVAL PROCESS AND REQUIREMENTS will be restated in its entirety and the proposed contract will be published and available for reprint.

  • Submit Documents:
    • Written request to be approved
    • West Virginia Business License
    • Current West Virginia Contractor's License
    • West Virginia Workers Compensation (The contractor cannot opt out of coverage.  Every person assigned to a job site must be covered by WV Workers Compensation).
    • IRS Form W-9
    • Three (3) Customer References
